A child of Portuguese parents was born in Hong Kong. The Portuguese parents would like to register the child with the Portuguese Civil Registry.
Both parents have to a sign a declaration form (in Portuguese) before the notary public in Hong Kong if the couple is located in Hong Kong. The notary public is to witness the signatures of the parents and sign and affix official seal on the declaration form.

Things to bring to the appointment:
1. The original declaration for notarization
2. HKID and passports
3. Portuguese citizen cards
4. Birth certificate of the child
5. Address proof within recent 3 months (e.g. utility bill, bank statement)
