Case study
If your child is a minor (i.e. person under 16 years old) applying for a US passport and only one of the parents / guardians can appear for the application process, then the non-applying parent / guardian must sign the Passport Parental Consent, or Statement of Consent (Form DS-3053) in front of a Notary Public and present a piece of government-issued photo identification. This identification must be photocopied (front and back) and submitted along with the form.
Both parents / guardians must authorize the issuance of a minor’s passport and must provide the proper documentation of citizenship and guardianship.
The non-applying parent/ guardian will have to bring the following to the Notary Public Services Appointment:
1) The completed Form DS-3053 (which has to be signed by you in person before Notary Public)
2) Original of your valid passport / HKID
3) Copy of the birth certificate of your child
4) Copy of the valid passport of the applying parent / guardian
5) Copy of your address proof (e.g utility bill of recent 3 months)
